Parents and athletes must complete the Winter Term Program Application to be considered for acceptance. 

1. Complete the Winter Term Athlete Application.
2. Request copy of official School Transcripts from sending school. Please ask your guidance counselor to mail a copy to the address below.
3. Request two confidential Teacher Recommendations. 

Download the Teacher recommendation form provided below. Recommendations can be emailed or mailed to the address below. The recommendation can be editing using the Adobe Acrobat Reader.
